Please go to the CHE ABC program building to register.

The following items will also be needed:

• Child Physical Form

• Birth Certificate or Hospital Record

• Child's Social Security Card

• Child's Insurance Card

• Child's Pick-up List

• Immunization Record

• USDA Food Program Eligibility Form

• Proof of Income: Total Family Income

All Parents/Guardians are required to have for ABC students:

1. 2 Change of clothes available for their child/children each day.

2. 2 blankets for naptime.
